چکیده
A MODIFIED NISSEN TOUPET PROCEDURE FOR THE TREATMENT OF GASTROESOPHAGEAL REFLUX DISEASE AND HIATAL HERNIA: HOW I DO IT (Abstract): The laparoscopic fundoplication became the standard treatment for gastro-esophageal reflux disease and hiatal hernia. Nissen and Toupet techniques are the most used surgical procedures. Which of these techniques is superior is a subject of debate. The Nissen procedure appears to be associated with a higher rate of postoperative morbidity (postoperative dysphagia, gas bloat syndrome). It is classically considered the Nissen procedure allows a superior reflux control compared with the Toupet operation. We present our laparoscopic technique which combines the two procedures and which was associated along the time with a low rate of postoperative morbidity and a good reflux control.

Only a minority of patients with gastro-esophageal reflux disease (GERD) are offered a surgical option. This is mostly due to the fear of potential side effects, the variable success rate, and the extreme alteration of gastric anatomy with the current gold standard, the laparoscopic Nissen fundoplication. It has been reported that laparoscopic Toupet fundoplication (LTF) and laparoscopic sphinc...

BACKGROUND One of the most frequent complications after laparoscopic antireflux surgery is estimated to be the intrathoracic herniation of the wrap into the chest. Therefore, in up to 5% of patients, revisional surgery is necessary. HYPOTHESIS Patients who undergo laparoscopic refundoplication for postoperative intrathoracic wrap herniation using a circular polypropylene mesh for hiatal closu...
متن کاملChronic Asthma and Gastro-Esophageal Reflux Disease: The Treatment Plans
Gastro-esophageal reflux disease (GERD) regularly occurs when stomach acid moves up from the stomach into the esophagus. GERD might be associated with chronic asthma symptoms such as coughing and breathlessness. According to several studies on children and adults, GERD is proven to have a close relationship with asthma. Medication treatment via proton-pump inhibitors (PPIs), such as Omeprazole,...
متن کاملLate Evaluation of Patients Operated for Gastroesophageal Reflux Disease by Nissen Fundoplication
Background: Surgical treatment of GERD by Nissen fundoplication is effective and safe, providing good results in the control of the disease. However, some authors have questioned the efficacy of this procedure and few studies on the long-term outcomes are available in the literature, especially in Brazil. Aim: To evaluate patients operated for gastro-esophageal reflux disease, for at least 10...
متن کاملThe importance of the mesh shape in preventing recurrence after Nissen fundoplication.
Gastro esophageal reflux disease (GERD) is the most common gastrointestinal disorder and often is associated with hiatal hernia (HH). Nissen fundoplication is the most common surgical treatment method. Despite surgical treatment, recurrence rate is still high. In this study, we aimed to identify the importance of the mesh shape in preventing recurrence after Nissen fundoplication. A hundred twe...
